Agreement to collect and use of personal information

National efficacy evaluation center for the health products targeting arthritis and immune diseases (’CAID’) is committed to ensuring the privacy and confidentiality of your personal information. In accordance with the Personal Information Protection Act, we would like to obtain your consent. Please read the following points carefully before checking your consent.

1. ① Purpose of collection and use of personal information: The collected personal information is used for the purpose of providing efficacy evaluation service, responses to inquiries, and the collected personal information is not used for other purpose than this collection and use purpose.
2. ② Personal information items to be collected: name, affiliation, position, e-mail address, phone number
3. ③ Retention period of personal information: In accordance with the Personal Information Protection Act, personal information will be deleted after one month. (However, service users will be deleted after service completion.)
4. ④ You have the right to refuse consent to the collection and use of personal information, which is necessary for your use of the service, but if you decline your consent, you will not be able to use the service.
